Hospital Discharge: This service assists with navigating the hospital discharge, bringing your loved one home, designing services and obtaining resources for after-care, and care management to prevent re-hospitalizations. We assist with Medication Management, dietary changes, caregiver scheduling, home health and hospice services, attend follow up appointments and monitoring of recovery.
Care Plan Development and Monitoring: Seniors living alone or away from family often need support but don't know where or how to receive it, unless an emergency arises. Care First Resources provides that piece of mind, knowing someone is checking in on them on a regular basis, monitoring for changes in health or decline, following up on home management services (private caregivers, yard maintenance, bill paying), and of course, support when and if a health crisis arrives. This is a great way for distant family members to have peace of mind, knowing that someone is checking on their loved one and keeping them in the "loop" as things arise.
Vacation Care: So many families take care of their aging or chronically ill loved ones. At Care First Resources, we understand the need for a vacation. We provide coverage for family caregivers when they have to (or want to) leave town. This is usually short term coverage and can assist with continuity of care while you are away.
In Home Care and Placement Services: As a reminder, we offer complementary (FREE!) assessments for families seeking in home care or placement services. We provide a short evaluation and assist you in determining what service might be most appropriate and help you navigate the process of selecting the right service agency or facility.
